ABOUT MIRA
Mira is the AI operating system for orthopedic practices. The founding team are repeat founders — ex-Amazon ML, ex-CTO of an a16z-backed company, and an HSS-trained orthopedic surgeon with deep expertise in scaling health tech. We're backed by Google DeepMind exec and Anthropic/OpenAI healthcare technical staff, YC founders, and the surgeons we build for. We're also supported by clinical advisors trained at HSS, Johns Hopkins, Stanford, UCSF, Harvard, Cleveland Clinic, Mayo Clinic, UT Southwestern, and Rush.
Our agents run the whole loop: documentation, intake, a 24/7 patient voice concierge, billing and RCM, contract data, and automation agents that log into legacy EMRs.
We're growing incredibly quickly (doubling revenue every month) and would love someone ambitious and operationally amazing to join the team.
THE ROLE
As a founding engineer, you'll own entire product surfaces — from the first conversation with a surgeon to the feature running in their clinic. You'll be embedded with our customers (orthopedic surgeons, billers, and practice managers), watching real workflows, finding the bottleneck, and shipping the fix the same week. This role is in-person, in NYC.
This is not a "build features off a backlog" role. You decide what to build, you build it, and you put it in front of users. We're a small team of builders who move fast and ship products people genuinely love.
WHAT YOU'LL DO
Own products end to end across our suite — Scribe, Dictate, Intake, the voice Concierge, EMR-automation agents, billing/RCM, and the contract-data layer — from idea to production, across the full stack.
Ship AI agents that do real clinical and revenue work: drafting op notes, working claims, clicking through legacy EMRs, answering patient calls, surfacing payer rates.
Build directly with surgeon customers, turning what you see in the clinic into shipped product the same week.
Set the technical bar early — the patterns, architecture, and quality the next ten engineers will build on.
NON-NEGOTIABLES
Strong TypeScript/JavaScript and React, with real fluency across the modern web stack and the Next.js App Router.
You're genuinely AI-native — you build with LLMs daily and have shipped AI features users rely on, not just demos.
Comfortable owning production on AWS: you can take something from idea to deployed and monitored without hand-holding.
High agency - drive to make a real difference and to know your work mattered.
Relentless curiosity and the conviction that no problem is unsolvable.
Autonomy and ambition. Even if you didn't work for us, you'd be making an impact somewhere.

COMPENSATION
Base salary of $150K–$180K, plus meaningful founding-engineer equity.
